Dry needling — peripheral and central considerations

Dry needling is a common treatment technique in orthopedic manual physical therapy. Although various dry needling approaches exist, the more common and best supported approach targets myofascial trigger points. This article aims to place trigger point dry needling within the context of pain sciences. From a pain science perspective, trigger points are constant sources of peripheral nociceptive input leading to peripheral and central sensitization. Dry needling cannot only reverse some aspects of central sensitization, it reduces local and referred pain, improves range of motion and muscle activation pattern, and alters the chemical environment of trigger points. Trigger point dry needling should be based on a thorough understanding of the scientific background of trigger points, the differences and similarities between active and latent trigger points, motor adaptation, and central sensitize application. Several outcome studies are included, as well as comments on dry needling and acupuncture.

Physiotherapy and acupuncture practice in Denmark

Abstract

Implementing acupuncture in physiotherapy requires careful consideration of ethics, safety and hygiene, rationale of the clinical application, and principles of evidence-based practice. The ethical and evidencebased principles in particular should be borne in mind when implementing acupuncture in the physiotherapeutic armamentarium.

Acupuncture based on the theories of traditional Chinese medicine and obsolete Western theories has to give way to the tremendous amount of new scientific knowledge from basic science and clinical studies.Supported by science, acupuncture is a form of afferent stimulation with clinically relevant effects. Clinical studies showing these relevant effects are primarily based on modern medical acupuncture principles, drawing on modern medical diagnoses and neurophysiologic explanations. Medical acupuncture has successfully been integrated from an academic perspective in modern Danish physiotherapy practice by the Subgroup for Acupuncture under the Danish Association of Physiotherapists.

Clinical research confirms that acupuncture as an adjunct in the conservative treatment of musculoskeletal nociceptive pain conditions can be a worthwhile supplement. Osteoarthritis of the hip or knee, spinal pain, and headache/migraine are examples where modern medical acupuncture could be evaluated as a relevant and evidence-based treatment.

Nyt elektroakupunktur-apparat

Vores ”gamle” el apparat til akupunktur er taget ud af produktion og vi har i faggruppen været på udkig efter et erstatningsapparat. British Medical Acupuncture Society har testet forskellige apparater og er fundet frem til, at AS Super 4 Digital er et godt alternativ. De afholdt et introduktionskursus på St. James Hospital i Dublin. DFFAS var repræsenteret ved faggruppens formand, Ole Andersen.

DFFAS vil fremover bruge denne el-stimulator på kurserne. Apparatet er nemt at betjene og koster kun ca. det halve af forgængeren. Der er planer om at lave et endags kursus i el-akupunktur til efteråret. Apparatet kan købes gennem www.tcm-shop.dk
